Weight loss can absolutely make the lower face look more saggy or hollow, especially after 40. But it’s not because you did anything wrong… and it doesn’t mean your skin is “ruined.”

It usually comes down to a few simple changes happening at the same time:

  • Loss of cheek volume: The cheeks help support the lower face. When that volume shrinks, the area around the jawline can start to drop.
  • Less skin elasticity after 40: Skin doesn’t “snap back” as easily, so when fat is lost, the skin may look looser.
  • Gravity becomes more noticeable: Without as much structure underneath, everything tends to settle lower.
  • Faster weight loss = more noticeable change: Rapid loss (including with things like Ozempic) gives the skin less time to adjust.

This is the part most people don’t realize.

The upper face—especially the cheeks—acts like a natural support system for the lower face. When that area is fuller and slightly lifted, it helps keep everything below it looking smoother.
